Optional Nutritional Evaluation Survey: $40 for 40 min
The Nutritional Evaluation Survey is a brief introductory evaluation to determine if you need more work, or what my expertise can help you with.
A. Intake and Assessment: $250 (two sessions one week apart):
- 1:15 hour: Nutritional health profile, 3-5 day food journal, and health history intake forms: Assessment may include: readings of laboratory findings, subjective reports, body palpation, muscle testing, and tongue pulse diagnostics, functional evaluation, neurological and neuro-lingual testing, Nutritional Response Testing, Quantum Reflex Analysis, Chapman Reflex test, etc. for more information. All of this will be reviewed in preparation for the report of findings appointment.

- 45 min: Report of findings and treatment plan/protocol: Here we will go over all findings and develop a treatment protocol and plan to address needs in terms of detoxification, nutrition and lifestyle.
B. Follow up options
- 60 min: $95 First assessment, adjustment to protocol, and re-evaluation 3-4 weeks after assessment. Re-Eval form here
- 45 min: $85 Extended check-in change of circumstance
- 30 min: $70 Brief check-in and testing
